Impact / Expected Impact

The project will reduce the number of clinic visits and enhance patients’ safety by a mobile health platform with wearable devices to remotely monitor the conditions of patients with chronic heart failure.  The target beneficiaries are mainly low-income patients with acute heart failure who are about to be discharged from the hospitals.
