This project aims to develop a new approach to engage volunteers and support vulnerable patients remotely by utilising a mobile application and integrating a volunteer service to organise medical history (provide assistance in summarising and organising patient information/medical history in written English/Cantonese/Putonghua) to help patients in health professional consultations, so that they can have equal access to healthcare resources and support.
