Developing a community-based chronic disease management model to support the elderly and low-income residents in Hong Kong affected by chronic obstructive pulmonary disease (COPD) or fatty liver disease, while strengthening post-discharge follow-up and self-management to improve their quality of life
This venture aims to develop a community-based chronic disease management model to support the elderly and low-income residents in Hong Kong affected by COPD or fatty liver disease. The project also strengthens post-discharge follow-up and self-management by working with the hospital and NGO, aiming to improve the quality of life of the target beneficiaries.
The project will deliver health management programs, community-based health assessments and education campaigns on chronic disease management. Milestone-based research for project evaluation will be conducted by Saint Francis University.
