Identifying elderly with a high risk of stroke and fall for referrals to follow-up actions by medical professionals

 

 

Adopting an algorithm-based software, which is based on a biomedical database consisting of 180 000 patient data, the venture aims to partner with day care centres/ homes for the elderly and help identify elderly with a high risk of stroke and fall for appropriate referrals to medical professionals.

The venture will conduct screening with around 1 000 elderly. Of them, around 300 who are of high risk are expected to be referred to medical professionals for follow-up actions. Besides, at least one workshop relating to fall prevention/ handling will be conducted for each partnered centre.
