    Providing health assessments and follow-up care to grassroots older adults and single parents through training individuals from low-income families to become community health workers

      

      

    The venture will train a total of 100 trainees from low-income families, including 20 older adults and single parents, to become community health workers (CHWs). With the assistance of healthcare professionals, the CHWs will provide a total of 1 500 hours of health assessments and follow-up care service for the grassroots community.

    The CHWs will provide Health Need Assessment to 500 older adults and single parents from low-income families, among whom 250 will receive personalised lifestyle modification plans and three months of follow-up care.
