Providing grassroots elderly with free services and support related to making medical instructions and end-of-life arrangements in advance

 

The traditional means of creating Advance Care Planning (ACP) documents (i.e. Will, Advance Directive and Enduring Power of Attorney) generally involves a costly, complicated and time-consuming process. Through the development of a mobile app and provision of affordable witnessing services, the venture aims to promote the awareness, understanding and adoption of ACP documents.

Seminars on ACP documents and the use of the mobile app will be organised at elderly centres and non-governmental organisations for 500 elderly and their family members. Two hundred free mobile app accounts will be provided to grassroots elderly for them to create their ACP documents which will be legally acceptable upon having witnessed by doctors and/ or lawyers. In addition, at least 100 grassroots elderly will enjoy the witnessing service for free.
